3. Usted desea solicitarle al Juez adoptar una persona adulta que en la actualidad es o anteriormente fue su hijastro(a), su sobrino(a), o nieto(a); O

4.Usted es un padre de acogida y quiere pedirle al Juez adoptar a una persona adulta que fue puesta bajo su cuidado cuando era un menor, y con quien ha mantenido una relación familiar por cinco años o mas; Y

5.El adulto que usted esta pidiendo adoptar esta de acuerdo con la adopción, Y

6. (si corresponde) El esposo/a de quien quiere adoptar y/o la persona a ser adoptada esta de acuerdo con la adopción, Y

7. (si corresponde) Usted comprende que cualquier cambio de nombre resultante de esta adopción no afectara sus derechos ni tampoco le eximirá de cualquier obligación o responsabilidad incurrido bajo su  nombre actual

You may use this form if the following factors apply to your situation:

• You are a resident of the county where you are filing this request, AND,
• You want to ask the Court to change the name of your family (yourself and/or your spouse and/or minor children), AND,

• You are prepared under penalty of perjury to inform the Court whether you have ever been convicted of a felony and whether there are any pending charges against you for a felony or other offense involving false statements or misrepresentation of identity, AND,

You may use this form if the following factors apply to your situation:

• You want to ask the Court to change the name of a person under 18 years of age who resides in the county where you are filing this request, AND,
• You are that child's parent or guardian.

DO NOT USE THESE FORMS TO ADD A FATHER'S NAME TO BIRTH CERTIFICATES OR ESTABLISH PATERNITY.

You may use this form if the following factors apply to your situation:

• You are a resident of the county where you are filing this request, AND,

• You are an adult 18 years or older, AND,
• You are the parent of at least one minor child, AND,
• You are prepared under penalty of perjury to inform the Court whether you have ever been convicted of a felony and whether there are any pending charges against you for a felony or other offense involving false statements or misrepresentation of identity, AND,
• You understand the name change will neither harm your rights nor release you from any obligations or liabilities incurred under your current name, AND,
• You are not changing your name to that of another person for the purpose of committing any crime or furthering any offense involving fraud or misrepresentation of identity.

You may use this form if the following factors apply to your situation:

• You are a resident of the county where you are filing this request, AND,
• You want to ask the Court to change your name, AND,
• You are an adult 18 years or older, AND,
• You do not have a minor child(ren), AND,
• You are prepared under penalty of perjury to inform the Court whether you have ever been convicted of a felony and whether there are any pending charges against you for a felony or other offense involving false statements or misrepresentation of identity, AND,
• You understand the name change will neither harm your rights nor release you from any obligations or liabilities incurred under your current name, AND,
• You are not changing your name to that of another person for the purpose of committing any crime or furthering any offense involving fraud or misrepresentation of identity.

You may use these forms and instructions in this packet if...

• You need to have payment of filing fees and/or other court fees and costs deferred, that is, you need to pay later, AND,
• You understand that since waivers, which excuse payment completely, are rarely granted at the beginning of a case, if your request for deferral is granted you will later receive either a bill for the full amount or a payment plan from the Court, AND,
• You understand that when you receive that bill or payment plan, you may then file a "Supplemental Application" to request more time to pay or for the court to waive payment of fees and costs completely, AND,
• You understand that filing these forms does not guarantee your fees will be deferred or waived, but that deferral or waiver will be granted if you meet the requirements established by law.
• You understand that IF your request for deferral is granted, a charge of $30 will be added to your deferred fees.

You may use this form if the following factors apply to your situation

• You may use the forms and instructions in this packet if...
  
  • Your case was handled in the Superior Court of Arizona in Yuma County, AND
  • You have read the set aside conviction statutes (A.R.S. § 13-907 and those that follow), and you are eligible under the statutes.
  • You are asking the Court to reconsider a denial of a previous application.
• DO NOT USE THE FORMS in this packet if:
  
  • Your conviction was from a justice, city or magistrate court. Contact that court for further information.
  • You are looking for "expungement." Arizona does not expunge criminal records. An order setting aside the judgment of conviction pursuant to A.R.S. § 13-907 DOES NOT REMOVE the conviction from the public record. Arizona law deems this a matter of public protection.
