You may use this form if the following factors apply to your situation:
- You want to ask the Court to change the name of a person under 18 years of age who resides in the county where you are filing this request, AND,
- You are that child's parent or guardian.
DO NOT USE THESE FORMS TO ADD A FATHER'S NAME TO BIRTH CERTIFICATES OR ESTABLISH PATERNITY.
