Prior to filing for a name change you will need the following:

• Time and patience
• Photo ID (if available)
• Social Security Number
• Changing a the name of a child
  
  • Birth Certificate
  • Signatures of both parents
    
    • If the parents are not married, the individual filing has to notify the other 30 days in advance.
• History and case numbers of previous or pending criminal convictions (if applicable)
