A treatment plan that does not require continuous inpatient hospitalization.

A testamentary instrument that is executed and maintained on a tangible medium and that is executed in compliance with Arizona Revised Statutes, Section 14-2502 or 14-2503.

Pay on death designation or "POD" means the designation of:

• A beneficiary in an account payable on request to one person during the lifetime of the person and on the death of a person to one or more beneficiaries or to one or more persons during their lifetimes and on the death of all of them to one or more beneficiaries.
• A beneficiary in an account in the name of one or more persons as trustee for one or more beneficiaries if the relationship is established by the terms of the account and there is no subject of the trust other than the sums on deposit in the account, whether or not payment to the beneficiary is mentioned.
  
  

Payment or "payable" as it relates to sums on deposit includes withdrawal, payment to a person or third person pursuant to a check or any other request and a pledge of sums on deposit by a person, or any setoff, reduction or other disposition of all or part of an account pursuant to a pledge.

Sheriffs of counties, constables, marshals and policemen of cities and towns.

A severe mental disorder that meets all the following criteria:

• If not treated has a substantial probability of causing the person to suffer or continue to suffer severe and abnormal mental, emotional or physical harm that significantly impairs judgment, reason, behavior or capacity to recognize reality.
• Substantially impairs the person's capacity to make an informed decision regarding treatment, and this impairment causes the person to be incapable of understanding and expressing an understanding of the advantages and disadvantages of accepting treatment and understanding and expressing an understanding of the alternatives to the particular treatment offered after the advantages, disadvantages and alternatives are explained to that person.
• Has a reasonable prospect of being treatable by outpatient, inpatient or combined inpatient and outpatient treatment.
  
  

A record that grants an agent authority to act in the place of a principal.

A presently exercisable general power of appointment other than a power exercisable either:

• By a trustee and limited by an ascertainable standard.
• By a person other than in a fiduciary capacity and only on the consent of the trustee or a person holding an adverse interest.

The review of each application requesting court-ordered evaluation, including an investigation of facts alleged in such application, an interview with each applicant and an interview, if possible, with the proposed patient. The purpose of the interview with the proposed patient is to assess the problem, explain the application and, when indicated, attempt to persuade the proposed patient to receive, on a voluntary basis, evaluation or other services.

A form established by a court or the rules of the administration in accordance with the laws of this state.

Property held in trust for distribution to a remainder beneficiary when the trust terminates, or any person to whom a fiduciary as such owes an obligation.

Anything that may be the subject of ownership, whether real or personal, legal or equitable, or any interest in anything that may be the subject of ownership.

A proceeding under Arizona Revised Statutes, Section 14-5401 to determine that a person cannot effectively manage or apply his estate to necessary ends, either because he lacks the ability or is otherwise inconvenienced, or because he is a minor, and to secure administration of his estate by a conservator or other appropriate relief.

A registered nurse practitioner as defined in Arizona Revised Statutes, Section 32-1601 who has completed an adult or family psychiatric and mental health nurse practitioner program and who is certified as an adult or family psychiatric and mental health nurse practitioner by the state board of nursing.

A licensed physician who has completed three years of graduate training in psychiatry in a program approved by the American medical association or the American osteopathic association.

A beneficiary who, on the date the beneficiary's qualification is determined:
Is a distributee or permissible distributee of trust income or principal. Would be a distributee or permissible distributee of trust income or principal if the interests of the distributees described in subdivision (a) of this paragraph terminated on that date. Would be a distributee or permissible distributee of trust income or principal if the trust terminated on that date.

A person who fulfills the requirements of Arizona Revised Statutes, Section 14-2520.

An organization under contract with the administration to coordinate the delivery of mental health services in a geographically specific service area of the state for eligible persons.

A person who practices registered nursing and who holds a registered nurse license issued pursuant to this chapter or pursuant to a multistate compact privilege.

A person who originates or transfers a security title by registration and includes a broker who maintains security accounts for customers and a transfer agent or other person who acts for or as an issuer of securities.
