Prior to filing for a child support you will need the following:
- Time and patience
- Names and dates of birth of children
- Gross income of both parents
- Spousal maintenance paid or received by either parent
- Court-ordered child support for other children (if applicable)
- Medical, dental, and vision insurance costs for children
- Educational expenses
- If parenting time (custody) is not equal, the amount of days per year each parent is with the youth
- Court-ordered arrears paid by the noncustodial parent
If submitting a Request for Title IV-D Child Support Services with the Department of Child Support Services, you will need the following:
- Application is to be completed using BLACK ink
- Valid identification
- A copy of
- Birth certificates for all children involved
- Social Security cards
- Marriage licenses (if applicable)
- Pertinent death certificates
- All court orders/modifications that are related to the child support request
- All payments, related to your case, that have been made outside of ATLAS
- Current or last known location information for the other person
